Trade Secret and Beauty Express Style Club and Style Club Select Terms and Conditions: Subject to the terms and conditions in this application, members of Style Club and Style Club Select will receive Reward Cards for purchases of qualifying products and salon services at participating Trade Secret/Beauty Express locations during the term of the respective program. Reward Cards are redeemable for qualifying products and salon services before their expiration dates at participating Trade Secret/Beauty Express locations. I. Membership: a. Any customer of Trade Secret/Beauty Express age 13 and older (parent or legal guardian must provide written consent for applicants under age 13) may apply to become a member of Style Club or Style Club Select by completing all required fields and submitting this application at participating Trade Secret/Beauty Express locations. Store employees are not eligible to participate in the program. Style Club Select applicants only are required to pay a one-time membership fee of $9.95 U.S./$10.95 CAN (fee subject to change) in return for additional program benefits. Trade Secret/Beauty Express will then assign a card with a Style Club or Style Club Select account number to the individual accepted for membership in the respective program. b. In order to receive Reward Cards, members must keep their personal information up to date by calling Customer Service at 1-800-STYLE4U. Trade Secret/Beauty Express may also obtain change of address updates from other sources such as the U.S. Postal Service. II. Participation: a. The Style Club or Style Club Select card must be provided by the member at the time of purchase to get credit for the purchase. Only members who purchase the products/services will receive credit for points (even if paying for other or additional Style Club or Style Club Select members). In addition, Trade Secret/Beauty Express may credit a purchase to a member's account if the member provides the original receipt and account number to a participating location or faxes the receipt and member's account number to a designated fax number. b. For every $1 in qualifying product or service purchases at participating Trade Secret/Beauty Express stores, excluding payment for any federal, state and local taxes, gift card purchases, reward card or other coupon redemptions, and membership fees, Trade Secret/Beauty Express will award 1 point to member's account. Exceptions may be allowed during specific promotions. c. Refunds, checks returned due to non-sufficient funds, or disputed credit card charges may be deducted from member's account at a rate equal to the original transaction. d. Employees of Trade Secret/Beauty Express (or their parent, subsidiary, or affiliated companies) are not eligible to participate. e. Purchases made at non-participating stores are not eligible for points. III. Awards: a. Accounts will be reviewed periodically. Accounts that have accumulated the then-current qualifying point total at the time of review (qualifying point totals are subject to change from time to time) will be awarded a Reward Card in a value to be determined by Trade Secret/Beauty Express in its sole discretion. Once an account has earned enough points to be awarded a Reward Card, all such points are deleted from the account and the point accumulation process starts over. Reward Cards will be awarded to qualifying account holders and mailed to the eligible account holder's address of record. b. Reward Cards must be used before the expiration date indicated on the card or the Reward Card will be forfeited. c. Points from multiple accounts may not be aggregated. d. Style Club and Style Club Select cards and Reward Cards have no cash value and are not exchangeable, returnable, refundable, or redeemable for cash. e. Style Club and Style Club Select cards and Reward Cards are not transferable and may not be offered for sale (including via any Internet auction site) and are void if transferred or sold or exchanged for cash or other consideration or if received or used in violation of these terms and conditions. IV. Additional Terms and Conditions: a. Trade Secret /Beauty Express reserves the right to terminate, cancel, modify, or restrict Style Club/Style Club Select or any aspect of the program at any time with or without notice. b. Membership in Style Club/Style Club Select may be revoked at any time if a member abuses the program, fails to follow any terms of the program, or makes any misrepresentation, any of which will affect eligibility for further participation in the program. V. Membership Expiration: a. Any account with no qualifying purchase for a period of one year or longer may be deemed inactive and the account and all points therein may be deleted. b. Eligible purchases and points reflected in Trade Secret/Beauty Express records shall be deemed correct and Trade Secret/Beauty Express reserves the right to determine the amount of Reward Cards to be awarded to eligible Style Club and Style Club Select card accounts and to correct or modify the Reward Cards at any time based on Trade Secret/Beauty Express records and calculation of account information.
